How to use it
- Fill in the medicines. Copy each medicine and its time from the label or your medication list. Ask your pharmacist if anything is unclear.
- Print at actual size. Choose “Actual size”, not “Fit to page”, so the boxes stay big enough to tick comfortably.
- Tick as you take them. Keep it next to your pill box. Bring it to appointments to show what really happened.
What is on the sheet

Can I edit it before printing?
The PDF has blank lines to fill in by hand. Nothing you write is sent anywhere.
What about medicines I take twice a day?
Use one row per dose time, for example Metformin 8:00 and Metformin 20:00, and put the time in the Time column.
