How to use it

  1. Write each reading. Put the date, the time and the numbers exactly as your device shows them. Ask your nurse or pharmacist which readings matter for you.
  2. Print at actual size. Choose “Actual size”, not “Fit to page”, so the boxes stay big enough to tick comfortably.
  3. Take it to appointments. Bring the page to show your doctor how the readings looked over the weeks.

What is on the sheet

  • Name and month lines at the top
  • A row for every reading, with date and time
  • Columns for blood pressure, pulse and glucose
  • Columns for meals, doses and a short note

Want it to remind you?

Questions

Which numbers are normal?

This page does not say. Your doctor or nurse knows your targets. Write them at the top if they gave you some.

Can I use it for one reading a day?

Yes. Leave the spare rows blank, or use one sheet per month.
