How to use it

  1. Write what you felt. Put the date and time, then tick how strong it was and add a short note in your own words.
  2. Print at actual size. Choose “Actual size”, not “Fit to page”, so the boxes stay big enough to tick comfortably.
  3. Take it to appointments. Bring the page so you do not have to remember. Ask your doctor or pharmacist what the notes mean.

What is on the sheet

  • Name and month lines at the top
  • A row for every entry, with date and time
  • Three strength boxes: mild, moderate, strong
  • Columns for pills taken, food, sleep and a note

Want it to remind you?

Oclo does the ticking from the notification and keeps the history for your doctor. Free, no account.

Questions

Does the diary tell me what a symptom means?

No. It only records what you felt. Ask your doctor or pharmacist what it means.

How often should I fill it in?

Whenever you notice something. Leave the spare rows blank, or use one sheet per month.
